Hospice Care Overview

Hospice Ward Team Members

Hospice and palliative care at our hospital is provided by five major interdisciplinary teams, delivering patient-centered care that encompasses the whole person, whole family, entire care process, full team approach, and the broader community—the “Five Wholes” of care.

The Hospice Ward is located on the 6th floor of the Third Medical Building and was established in October 2014. It currently has 12 beds. Deeply rooted in the local community along the Taosan (Provincial Highway 3) corridor in the Taoyuan–Hsinchu–Miaoli region, it is a comprehensive hospice care facility serving this area.

The ward has established a green channel for hospice and palliative emergency care, extends services into mountainous and rural areas, and integrates community-based and home care services. By providing culturally sensitive end-of-life care tailored to diverse populations, we aim to help individuals achieve a peaceful, dignified, and fulfilling end of life.


Services

Advance Care Planning (ACP) consultation and Advance Decision (AD) signing

-Before the Appointment

  • Must have full legal capacity
    (Age 18 or above, or age 16 and married)

  • Must have a National Health Insurance (NHI) card

  • Please call to make an appointment between 08:00–17:00

-ACP Consultation (Pre-ACP)

  • Self-paid consultation hours:
    Monday to Thursday afternoons
    (Subject to the monthly outpatient announcement)

  • Fees:

    • 1 person: NT$3,000

    • 2 people together: NT$1,500 per person

    • 3 or more people: NT$1,000 per person

    • Registration fee: NT$100

  • Completion method:

    • Complete forms on-site after consultation (30–60 minutes), or

    • Upload forms online (must be completed within 3 months)

      • If exceeding 3 months, an additional NT$200 upload fee applies

  • Healthcare agent:
    May be appointed or not (staff will provide further explanation)

-During the ACP Consultation (Approximately 60–90 minutes)

  • Required participants:

  1. The declarant (the person themselves)

  2. At least 1–2 second-degree relatives
    (Spouse must attend if married)

  3. Appointed healthcare agent (if designated)

-Completion / Upload of Advance Medical Decision (AD)

  • Requires two witnesses (second-degree relatives),
    or notarization by a notary public

  • Medical institution verification stamp

  • Registration recorded on the NHI card and becomes effective

  • Advance Medical Decision (AD) may be modified in writing

    • Re-issuance fee for changes: NT$200


Advance Care Planning Consultation Process

How Is an Advance Medical Decision (AD) Activated?

-When one of the specific clinical conditions is suspected:

  • Diagnosis by two specialist physicians

  • Confirmation by a palliative care team in a second consultation

-If one of the specific clinical conditions is met:

  • Terminal illness

  • Irreversible coma

  • Permanent vegetative state

  • Severe dementia

  • Other severe conditions officially announced by the central authority

-According to the Advance Medical Decision, the patient may accept or refuse:

  1. Life-sustaining treatment

  2. Artificial nutrition and hydration

-Transition to:

  • Palliative care

  • Hospice care
