Required Documents for Medical Visits
Please bring the following documents when visiting the hospital:
  • National Health Insurance (NHI) IC Card
  • National Identification Card (For children under 14 without an ID card: Household Registration Certificate)
Other applicable documents (not required for standard NHI patients)
  • Veterans ID (for veterans)
  • Hospital Staff ID (for hospital employees and dependents)
  • Child Health Handbook and Household Registration Certificate
  • Maternity Health Booklet (for pregnant women)
  • Disability Certificate or Catastrophic Illness Card
