Hospitalization Information

1. Check-in Location:
Inpatient Services Counter, 1st Floor, First Medical Building.
2. Required Documents:
(1) All patients: National Health Insurance (NHI) IC card and an identification document (a copy is acceptable).
(2) Veterans or dependents of veterans: Veteran ID card or Dependent of Veteran ID card.
(3) Foreign nationals: Alien Resident Certificate (ARC).
(4) Children: Household registration booklet and Hsinchu County Child Medical Subsidy Certificate (for Hsinchu County residents).
(5) Patients eligible for reduced fees: Relevant documents such as a Catastrophic Illness Certificate, Maternity Handbook, or Labor Insurance Occupational Injury Form.
3. Procedures:
(1) Document verification:
According to National Health Insurance Bureau regulations, if you do not have all required documents at the time of admission, you may still be admitted.
However, you must submit the missing documents within 10 working days from the admission date.
If you fail to do so, charges will be calculated as non-NHI (self-pay) rates.
(2) Consent form:
Please fill out the Inpatient Consent Form, indicating whether you agree to have your hospitalization information publicly disclosed.
4. Important Notes:
(1) Upon receiving notification of admission, please come to the hospital and complete the admission process before 5:00 PM on the same day.
(2) Bring any medications you are currently taking and inform the medical staff about them.
(3) The hospital provides the following items in the ward: patient gown, pillow, blanket, and TV (in the lounge area).
(4) Please bring your own personal hygiene and daily-use items, such as toothpaste, toothbrush, towel, cup, washbasin, urinal, bedpan, underwear, slippers, tissues or adult diapers, etc.
(5) Meal times: Approximately 7:00 AM, 11:00 AM, and 5:00 PM.

II. Inpatient Guidelines
1. General Rules for Inpatients:
(1) Smoking is strictly prohibited within the hospital. (The hospital enforces a strict no-smoking policy and provides smoking cessation support.)
(2) Do not cook, burn incense, or burn paper offerings indoors.
(3) Do not bring electrical appliances to the hospital.
If you need to use medical-related devices (e.g., air mattresses, electric wheelchairs), you must provide proof of manufacture and regular maintenance certificates.
The hospital will determine whether such devices are permitted.
(Mobile phones, chargers, tablets, laptops, and portable power banks are exempt.)
(4) The following personal electrical appliances are prohibited: rice cookers, induction cookers, heaters, heat lamps, electric blankets, fans, and hair dryers—anything that generates heat or consumes high power.
(5) Do not leave the hospital without permission. If you must go out, fill out a leave request form and obtain the doctor’s approval before leaving.
(6) Patients covered by National Health Insurance may not register for outpatient services under NHI while hospitalized.
2. Visiting and Overnight Stay Rules
(Except during special public health or epidemic control periods)
(1) Visiting hours: 10:00 AM – 8:00 PM (psychiatric, respiratory care, and intensive care units have separate rules).
(2) Companions: Only one companion may stay overnight with each patient; please register and obtain a Companion Pass at the nursing station.
(3) To prevent infection, children under 12 years old are not allowed to visit or stay overnight.
(4) Non-patients should not sleep in the patient’s bed or share the same bed.
(5) Companions should bring their own bedding.
(6) The hospital provides shared caregiver services; please inquire at the nursing station if assistance is needed.
3. Other Notes:
(1) Keep your identification and valuables with you. Single patients may ask the nursing station for safekeeping.
(2) Do not accept treatment from anyone not wearing a hospital uniform or staff ID badge.
(3) Do not leave food waste or garbage in the room, and avoid exposing hot food to prevent pests such as cockroaches. Help maintain cleanliness and hygiene.
(4) If you wish to order hospital meals, please inform the nursing station.
